Hartville Meadows, home to 32 developmentally disabled adults, opened on September 1, 1981. Located in the quiet village of Hartville, in Northeastern Ohio, Hartville Meadows is easily accessible from 1-77 and SR 619. The village and surrounding area provides a wide variety of activities and recreational events throughout the year.
Hartville Meadows is located on 5 acres and comprised of 21,000 sq. ft. Our Intermediate Care Facility features home like furnishings and a friendly atmosphere while also being completely handicapped accessible and fully sprinkled. Located under one roof, Hartville Meadows is divided into 4 living areas. Blue Valley and Green Willows are home to 8 men each. Red River is home to 8 women, while Yellow Springs is co-educational for 4 men and 4 women. Each living area has 4 bedrooms, 2 full bathrooms, a dining room and a living room. Every resident has a choice to furnish their own rooms or use the furnishings provided. All resident bedrooms and bathrooms are equipped with a ceiling lift system, which provides a virtual lift free environment. Other amenities that will make you or your loved one feel right at home are a large activity room, internet access, and a heated indoor pool.
Hartville Meadows is licensed by the Ohio Department of Mental Retardation and Developmental Disabilities and certified by the Ohio Department of Health to participate in the Medicaid Program. All services are covered under the Medicaid program including room and board. In following with State and Federal guidelines, each resident receives an individualized habilitation plan designed to aid in achieving their full potential. Specialized diets, 24 hour nursing services, physical, occupational, speech, and psychological services are areas that are tailored to each resident's specific needs. Emphasis is placed on individual needs with regard to training and treatment of all aspects of the individual's life, including independence in self care, social/recreational and cognitive skills. Residents and their families are integral members of the Interdisciplinary team and development of the individual habilitation plan.
Transportation:
Hartville Meadows provides three vans for transportation to medical appointments, community outings, special olympics, church, and resident vacations. Two of the vans are equipped with wheelchair lifts and are fully handicap accessible.

All services are delivered by trained staff under the direction of properly licensed and certified professionals.
Admissions:
Admissions to Hartville Meadows are approved after a complete review of the individual's needs, County Board and State recommendations, and the determination of the appropriateness of the placement. To be consider for admission to an ICF-MR (Hartville Meadows) the individual must need training in personal skills essential for privacy and independence. This list includes but is not limited to, toilet training, personal hygiene, dental hygiene, self-feeding, bathing, dressing, grooming, and communication of basic needs.

Since 1981, Hartville Homes, Inc. has operated facilities for persons with mental retardation and developmental disabilities. In August of 2000, these services were expanded to include five intermediate care facilities in West Lafayette, Ohio. These facilities, known as Lafayette Meadows, provide a high quality alternative to a more restrictive, institutional living environment for those persons who are unable to live without direct assistance.
Each facility is licensed by the Ohio Department of Mental Retardation and Developmental Disabilities and certified by the Ohio Department of Health to participate in the Medicaid Program.
All facilities are completely handicapped accessible and are equipped with emergency call and sprinkler systems.
The residential facilities operate 24 hours a day, with emphasis on the development of a home-like environment while assuring the health and safety of each individual.
